Forms

There are 3 forms that must be filled out by every participant to register for OAPI Summer Adventure Camps:

1.  Registration Form: word document or pdf

2.  Contact Information and Medical Form: word document or pdf

 3. Release and Assumption of Risk - Minor Form: word document or pdf 

 **You can either download and print at home or stop by the front desk at Gentile Hall for copies.

Other OAPI programs require the Contact Information and Medical Form (see #2 above) and the

Release and Assumption of Risk - Adult form.  This is required by ANY adult participating in an OAPI event or outing.
