Disability Services Forms and Documents (applicable to residence halls)

If applying for housing accommodations, please contact the Director of Disability Services Mary Kate Barbosa (mary.barbosa@maine.edu). If approved for the accommodation, the Office of Disability Services will notify the Residence Life Office of what accommodations are approved.
